If you turn off "Flow", then the brush is only applied when you move the mouse. With Flow on, if you just click and hold down, the brush keeps getting applied (smoothing etc)

Volumetric controls whether disconnected regions can be affected by the brush. It is a bit of a hack and doesn't always work. But normally VolumeBrush tools are applied to everything inside the sphere around the cursor. With this disabled, the system tries to find the patch connected to the point under the cursor, and only apply the brush to that part.

Hold Boundary controls whether the mesh boundary is allowed to move and/or be refined. With this enabled, the boundaries (blue lines) stay put.
